Renal cysts can be classified according to the Bosniak classification depending on their features. Type I cysts are simple cysts. Type II are the minimally complicated cysts. Type I and II can be ignored. Type II F are probably benign, but need to be followed. Type III and IV both are surgical lesions.
Background and Purpose: Bosniak III and IV renal cysts have low mortality potential, and little is reported regarding the feasibility and safety of managing such tumors by laparoscopy and its comparison with open surgery. We report on the experience with 37 complex renal cysts managed in the era of laparoscopy. Patients and Methods: A retrospective analysis of a prospective database from all
Saw them again in January and was told the same, "we will monitor." Not completely happy with this, as if you look on the internet, grade III Bosniak cysts are normally removed. 2017-03-16 · Bosniak II, IIF, and III. It is the panel’s opinion that when there is disagreement or doubt regarding the classification of a renal cyst, such case should be presented at a multidisciplinary meeting. (Level of evidence: 4; Recommendation: D) Description of Bosniak classification By means of the Bosniak classification, renal cystic lesions The mean largest diameter was 48.7 ± 28.8 mm. All lesions were multilobulated and septated. Of the 19 removed lesions, 16 (84%) were malignant, and 3 (16%) were benign (one inflammatory cyst including a nephrolith, one cystic nephroma and one atypical angiomyolipoma).

It is difficult to differentiate cysts Bocniak II and III even pathomorphologically. Cystic carcinoma was detected histologically in 8 Bosniak III cases.
(84%) in radiologically detected Bosniak category III cysts than it had been described before, [5-8,11] and draws a 4 ention to the diff erential diagnostic obstacles. Most of the radiologists believe that the malignancy rate of Bosniak category III cystic lesions is nearly 50%.[5] However, several studies demonstrated diff erent
Imaging and tests can help Dr. Gholami determine if a kidney tumor is likely benign or malignant. (eg, incompletely characterized but highly likely benign cysts), and (d) enables a greater proportion of masses to be placed into lower Bosniak classes. observing Bosniak III cystic masses and even some Bosniak IV cystic masses are very low during the initial 5-year pe-riod after diagnosis (12,34,38,39). In a pooled analysis by 2020-08-27 Bosniak III and IV type cysts are the surgical lesions. In the Bosniak III category the risk of aggressive tumor is minimal, so nephron sparing surgery is recommended.
Last month a CT scan was done for a stomach problem and of that cyst showed again as complex cyst. Pretty much the same except it grew from 2.2 cm to 3 cm still enhancing and still classified as a undetermined cyst, Bosniak 3 still. Renal cysts were classified by Bosniak, and this classification was modified in 2003, in order to attempt to narrow the large interval between type II and type III by the definition of IIF category [ 1 ]. However, none of the recent diagnostic modalities could clarify the grey-zone between type IIF and type III lesions (Figure 1).

Renal cell carcinoma (RCC) surveillance was not enhanced in the past 20 years. Bosniak classifications depend on different morphological modifications in renal cysts such as septa Background: Bosniak III and IV cysts have a high risk of malignancy and have traditionally been managed surgically. However, growing evidence suggests that many can be managed by active surveillance.

In our study, moderate to severe complications occurred in 19% (16/86) of patients who underwent surgical resection of a Bosniak cyst. A renal cyst is a fluid collection in or on the kidney. There are several types based on the Bosniak classification.

As Warren et al state; The MRI findings upgraded the lesion from Bosniak IIF to Bosniak III. As a result, the patient underwent a radical left nephrectomy and a biopsy, which revealed clear cell carcinoma. This case argues the limitations of the Bosniak classification and the value of using MRI at an earlier stage, especially with unusual circumstances such as a chronic history of enlarging cysts. The revised Bosniak system proposed that IIF cysts with fewer complex features (i.e., more similar to category II cysts) could be followed for two years after initial diagnosis, whereas those that were more complex (i.e., more similar to category III cysts) should be followed for up to four years . Pathological outcomes of category III cysts were recorded to determine the malignancy rate.
